Archdiocese of Chicago Education Survey
Our Catholic school team is currently engaged in an effort to assess the strengths and opportunities of our local Catholic schools. We recognize that parents consider many important factors when making school decisions for their children, and we respect the choices you have made. Understanding your views will help us improve our local Catholic School Program and will also enable us to understand the perspectives and needs of those who have not chosen Catholic school. We would greatly appreciate your help with our survey whether your children attend Catholic school or not. While the survey if offered by the Archdiocese of Chicago, the responses will be specific to each school so that we can better understand parent's view in our specific community. Transfiguration Elementary School Education Survey is for parents of children in preschool through grade 5. Frassati Catholic Academy Education Survey is for parents of children in 6th through 8th grades. To access the survey, please go to the following link: https://www.surveymonkey.com/s/SchoolSurveyEnglish or en Español: https://surveymonkey.com/s/SchoolSurveySpanish The survey will be available online until February 6. Thank you, in advance for your participation.
